Posterior Heel Spur (Pump Bump / Haglund’s Deformity)

A posterior heel spur, commonly referred to as a “pump bump” or Haglund’s deformity, is a bony enlargement that develops on the back of the heel where the Achilles tendon attaches to the calcaneus (heel bone). This condition often leads to irritation of the surrounding soft tissues, causing pain, redness, and swelling at the back of the heel.

The bump is frequently aggravated by shoes that rub against the heel—especially rigid dress shoes, high heels, and athletic footwear with a firm heel counter. Over time, repeated pressure can cause inflammation of the Achilles tendon insertion (insertional Achilles tendinitis) and surrounding bursae, leading to significant discomfort and difficulty with walking or exercise.

Symptoms of Posterior Heel Spur / Pump Bump

Patients commonly experience:

  • A visible bump at the back of the heel

  • Pain with shoe wear, especially closed-back shoes

  • Redness, swelling, or irritation over the heel

  • Tenderness at the Achilles tendon insertion

  • Thickened or irritated skin over the heel

  • Pain with running, walking, or climbing stairs

Causes and Risk Factors

This condition can develop from a combination of bone structure and mechanical stress, including:

  • High arches (increased heel pressure)

  • Tight or shortened Achilles tendon

  • Repetitive shoe irritation (“pump” shoes or rigid backs)

  • Biomechanical imbalance while walking

  • Overuse from running or sports

  • Genetic predisposition to bony prominence formation
